This application form is for family members (“applicant”) who have been granted standing at a coroner’s inquest where the deceased was a victim of crime or involved in a police-related incident. The applicant or their legal representative can complete this form to apply for reimbursement of eligible legal fees and expenses under either the Coroner’s Inquest Legal Fee Reimbursement Program or the Coroner’s Inquest Family Reimbursement Program. To be used by a local commander to make or renew an appointment under Part III of the Interprovincial Policing Act, 2009 or to advise that a requested appointment be denied.
To be used by extra-provincial commander to apply, under urgent circumstances, for the appointment of an extra-provincial police officer as a police officer under Part II of the Interprovincial Policing Act, 2009.
To be used by an appointing official to make an appointment under Part II of the Interprovincial Policing Act, 2009 or to advise that a request for an appointment be denied.

For costs incurred in transporting a body under a coroner's warrant. To be used by body transportation service providers in invoicing the Office of the Chief Coroner.
