This application form is for family members (“applicant”) who have been granted standing at a coroner’s inquest where the deceased was a victim of crime or involved in a police-related incident. The applicant or their legal representative can complete this form to apply for reimbursement of eligible legal fees and expenses under either the Coroner’s Inquest Legal Fee Reimbursement Program or the Coroner’s Inquest Family Reimbursement Program. Under subsection 11(3) of Christopher's Law (Sex Offender Registry), 2000, this form is to be filled out by police officers seeking to obtain a warrant to arrest a sex offender for the purpose of having him/her comply with the reporting requirement under Christopher's Law. This form is used if they are appearing in court to seek the warrant.
The AP061 is completed by members of the public when applying for the position of Auxiliary Constable.
