This application is to be used by licensed Security Guard and/or Private Investigator Agencies (Sole Proprietorship) to change their information (e.g., update head office and/or mailing address, Sole Proprietor contact information etc.) with the Private Security and Investigative Services Branch (PSISB). Instructions on how to successfully complete the application are included with the form.

This form is to be filled out by judges or justices of the peace if they decided to issue a warrant for the arrest of a sex offender for the purpose of having him/her comply with the reporting requirements under Christopher's Law. This form is a telewarrant.

This application form is for family members (“applicant”) who have been granted standing at a coroner’s inquest where the deceased was a victim of crime or involved in a police-related incident.
