Form 9 - Statement to Terminate Guardianship of Property by Summary Disposition under Section 73A person bringing a motion under the Substitute Decisions Act, 1992 to terminate a guardianship of the person may use this form if they want the motion to be dealt with by summary disposition. Subsection 73(1) of the Substitute Decisions Act, 1992 requires two statements. At least one statement must be completed by a designated capacity assessor using Form A. The other statement may be completed by a second capacity assessor (Form A) or by another person using this Form.
