The Public Notice of Application Form in subsection 0.4(1) of O. Reg. 244/97, under the Aggregate Resources Act, is for an applicant of an aggregate permit or licence to notify landowners within 120m of a proposed pit or quarry, to post the notice in a newspaper and to post on a sign at the proposed site (licence only).

This form (Part 2) is to be used by Commercial Bait Harvesters that have BHAs within the Lake Simcoe watershed and want to have their Emerald Shiners tested for VHS as part of the Lake Simcoe Emerald Shiner Testing Program, which would enable Emerald Shiners that test negative for VHS to be transported out of the Southern Bait Management Zone and into the Central BMZ and the Northeast Testing Program Area. Applicants need to submit both Part 1 and Part 2 of the application for screening and approval. This form (Part 2) provides details on each of the Holding Facilities identified in Part 1.

The production of salt by the solution mining method must be reported annually by the operator of a salt solution well.

This form is to be completed by any licence or permit holder applying to the ministry to make a change to their licence, permit, or site plan under sections 13, 13.1, 13.2, 30.1, or 37.2 of the Act. When notification is required, this form must be circulated with the amendment application package. Detailed submission information can be found on the form, depending on the nature of the amendment. Submit this form and any additional information to ARAapprovals@ontario.ca or if email is unavailable, Aggregates Section, Ministry of Natural Resources, 300 Water Street, Peterborough, ON K9J 8M5.

Within 15 days of an Agricultural Deer removal/Harassment Authorization expiring, the authorized property owner shall submit this Deer Removal Authorization Report Form to the MNR issuing office. A report is required even if no deer are taken.

Mechanism to apply for a bait harvest area (BHA). BHA's are an allocated portion of the resource and is the scale at which commercial bait harvesters are allowed to harvest baitfish and leeches. A commercial bait harvester can obtain as many BHAs as are available.

The Aggregate Resources Act (ARA) provides for the transfer of a licence or permit. A transferred licence or permit will allow the continued operation of the licensed or permitted site under the same terms, conditions and site plan requirements as the original licence or permit. Any changes or amendments to the site plan or licence/permit conditions are a separate process.

Under the Crown Forest Sustainability Act, 1994 and Ontario Regulation 167/95, you need a Forest Resource Processing Facility (mill) Licence if you want to construct and/or operate a mill and your mill consumes more than 1,000 cubic metres of forest resources in one year (regardless of the source). If you have a mill licence, you will need to get a new one where your existing licence is expiring/has expired or your mill: a) changes ownership, and/or b) changes productive capacity, and/or c) is converted to another mill type.
