This application is for individuals seeking to become head trappers on a registered trapline area. Information provided in this application will be used in the selection process where there are multiple applicants for a particular registered trapline.

This form is to be completed by any licence or permit holder filing an amendment to a site plan under sections 13(3.2) or 37.2(5) of the Act and section 7.2 of Ontario Regulation 244/97 under the Aggregate Resource Act (i.e., “self-filing”).

The MNRF Volunteer Application form outlines the applicant's knowledge, skills, qualifications relative to the individuals volunteer interests. The application form must be completed at the beginning of each new volunteer assignment.

Wildlife Custodian Authorization holders must submit this Annual Wildlife Rehabilitation Report for each calendar year by January 31st of the next year to the District Supervisor of their local ministry work centre.

A Trapping Licence, issued under the Fish and Wildlife Conservation Act, 1997 (FWCA) and its regulations, authorizes a person to hunt or trap furbearing mammals, black bear and other small game in the area designated on the licence and in accordance with the licence conditions (e.g., quotas) and regulations. This application form must be submitted to the local MNR Work Centre prior to a person being authorized to trap. This application also serves as a licence renewal form for authorization to trap in subsequent years.

The Public Notice of Application Form in subsection 0.4(1) of O. Reg. 244/97, under the Aggregate Resources Act, is for an applicant of an aggregate permit or licence to notify landowners within 120m of a proposed pit or quarry, to post the notice in a newspaper and to post on a sign at the proposed site (licence only).

Within 15 days of an Agricultural Deer removal/Harassment Authorization expiring, the authorized property owner shall submit this Deer Removal Authorization Report Form to the MNR issuing office. A report is required even if no deer are taken.

The Objection Form is a requirement in section 0.5(9) of O. Reg. 244/97, under the Aggregate Resources Act, to serve as a record of outstanding objections with respect to an application for an Aggregate licence, after the applicant has attempted to address all comments during the consultation period.

The Fish and Wildlife Conservation Act, 1997, and O.Reg. 668/98 (Wildlife in Captivity) requires a person who keeps specially protected raptors in captivity to submit an annual report of all of the specially protected raptors they possessed to the Ministry of Natural Resources and Forestry. The person must also keep a copy of the annual report for five years.

Application for an Aggregate Licence, Aggregate Permit or Wayside Permit under the Aggregate Resources Act

The Fish and Wildlife Conservation Act, 1997, and O.Reg. 668/98 (Wildlife in Captivity) requires that a person who keeps specially protected raptors or non-indigenous falconry birds in captivity keep a log book. Log books must be updated within 24 hours of any business transaction or event, or as soon as reasonably possible. Log book records need to be kept for five years and must be accurate.

Application for the Transfer of a Licence or Permit under the Aggregate Resources Act

This form allows individuals or incorporated organizations the ability to apply to purchase (sale and Crown patent), or rent, or use Crown land in the form of the following occupational authority types: lease, easement, licence, or land use permit. The occupational authority is a legal agreement between the ministry and tenant, and outlines who can use the land and for how long, the rents and fees and any associated conditions. The ministry’s primary focus for the disposition of Crown land is to: support the social and economic development of municipalities and Indigenous communities, and to provide land for infrastructure, such as energy facilities, utilities, and services. This is the Mandatory Season-End Report that is required to be completed by the holder of a Trapping Licence as per the Fish and Wildlife Conservation Act, 1997 and O.Reg. 667/98 (Trapping). Additional instructions can be found on the third page of this form.

Game Bird Hunting Preserves (GBHP) are areas in which pen-raised game birds are released into habitat to support additional game bird hunting, bird-dog training and field trial opportunities in Ontario within a predefined area. This licence is required to own and operate a GBHP, including keeping and releasing the authorized species of game birds on the preserve. Applicants must submit a complete licence application with all required supplemental information.

This is the Application for a General Falconry Licence. It must be completed and submitted to the local ministry work centre, along with any other required information, to apply for a General Falconry Licence. More information about falconry can be found at Ontario.ca/falconry.

If the holder of a Licence to Own and Operate a Train and Trial Area no longer wishes to operate their train and trial area during the period in which their licence is valid, they may request that the licence be transferred to another individual to own and operate that same train and trial area. The transferee (proposed new licence holder) and the transferor (current/outgoing licence holder) must complete this Application to Transfer a Licence to Own and Operate a Train and Trial Area form and submit to their local ministry work centre.

This form (Part 2) is to be used by Commercial Bait Harvesters that have BHAs within the Lake Simcoe watershed and want to have their Emerald Shiners tested for VHS as part of the Lake Simcoe Emerald Shiner Testing Program, which would enable Emerald Shiners that test negative for VHS to be transported out of the Southern Bait Management Zone and into the Central BMZ and the Northeast Testing Program Area. This licence is required for individuals to keep, propagate, buy and sell game birds in captivity for the purposes of supporting hunting (e.g., game bird hunting preserves, train and trial, municipal programs) or for personal consumption. Game birds kept under this licence cannot be released without additional authorization. Applicants must submit a complete licence application indicating which species of game birds they are seeking authorization to keep before acquiring any birds.
