A licence to Transport Live Fish is issued under authority of the Fish and Wildlife Conservation Act and is required to transport live fish or live spawn, other than baitfish, that have been taken from Ontario waters. This form allows an individual to apply for this licence. May be used to approve transport of live fish for the purposes of a fishing competitive event. Fish are then transported back into the originating lake. Issued for discrete dates and locations. Licence conditions may include holding tank water condition requirements, release of fish, record keeping requirements, record productions, among others.

This licence allows an individual or corporation to own and operate a Train and Trial Area. Part A of this form contains the application for the licence and Part B contains the required annual report. Those who wish to apply for this licence must have previously been issued a Licence to Own and Operate a Train and Trial Area, applications for new areas will not be accepted. Applicants must submit Part A and Part B of this form, as well as a copy of their site map.

Section 26 of the Fish and Wildlife Conservation Act, 1997 (FWCA) prohibits a person from using a dog to chase live game mammals or game birds during the closed hunting season for the purpose of teaching the dog hunting skills (training) or testing the dog’s hunting skills (field trial) unless the person has the authorization of the Minister. This application can be used to apply for an Authorization to Conduct Field Trials and Training which may be issued to authorize these activities during certain discrete periods of time outside of the hunting season for the game species being chased.

This form allows individuals or incorporated organizations the ability to apply to purchase (sale and Crown patent), or rent, or use Crown land in the form of the following occupational authority types: lease, easement, licence, or land use permit. The occupational authority is a legal agreement between the ministry and tenant, and outlines who can use the land and for how long, the rents and fees and any associated conditions. The ministry’s primary focus for the disposition of Crown land is to: support the social and economic development of municipalities and Indigenous communities, and to provide land for infrastructure, such as energy facilities, utilities, and services. This is the Application for an Apprentice Falconry Licence. It must be completed and submitted to the local ministry work centre, along with any other required information, including the Falconry Sponsor Information and Certification form, to apply for an Apprentice Falconry Licence. The Fish and Wildlife Conservation Act, 1997, and O.Reg. 668/98 (Wildlife in Captivity) requires that a person who keeps specially protected raptors or non-indigenous falconry birds in captivity keep a log book. Log books must be updated within 24 hours of any business transaction or event, or as soon as reasonably possible. Log book records need to be kept for five years and must be accurate.

This form is completed to collect the information required for the Ministry to consider the approval of tags for lake trout caught in Clearwater Bay, part of Lake of the Woods.

This form (Part 2) is to be used by Commercial Bait Harvesters that have BHAs within the Lake Simcoe watershed and want to have their Emerald Shiners tested for VHS as part of the Lake Simcoe Emerald Shiner Testing Program, which would enable Emerald Shiners that test negative for VHS to be transported out of the Southern Bait Management Zone and into the Central BMZ and the Northeast Testing Program Area. Applicants need to submit both Part 1 and Part 2 of the application for screening and approval. This form (Part 2) provides details on each of the Holding Facilities identified in Part 1.

This procedural guidance document is intended for those proponents with renewable energy projects proposed in whole or in part on provincial Crown land who have received a Feed-In Tariff (FIT) 5 energy procurement contract offer by the Independent Electricity System Operator (IESO).

This application is for individuals seeking to become head trappers on a registered trapline area. Information provided in this application will be used in the selection process where there are multiple applicants for a particular registered trapline.

This form is to be completed by any licence or permit holder filing an amendment to a site plan under sections 13(3.2) or 37.2(5) of the Act and section 7.2 of Ontario Regulation 244/97 under the Aggregate Resource Act (i.e., “self-filing”).

The MNRF Volunteer Application form outlines the applicant's knowledge, skills, qualifications relative to the individuals volunteer interests. The application form must be completed at the beginning of each new volunteer assignment.

This licence is required for individuals to keep, propagate, buy and sell game birds in captivity for the purposes of supporting hunting (e.g., game bird hunting preserves, train and trial, municipal programs) or for personal consumption. Game birds kept under this licence cannot be released without additional authorization. Applicants must submit a complete licence application indicating which species of game birds they are seeking authorization to keep before acquiring any birds.

This authorization is required for individuals, or corporations, to keep injured, sick or immature game wildlife or specially protected wildlife in captivity for the purpose of rehabilitating or caring for them. Those who are applying for this authorization for the first time should review how to become a wildlife custodian and contact their local ministry work centre before completing and submitting this application form.

The Public Notice of Application Form in subsection 0.4(1) of O. Reg. 244/97, under the Aggregate Resources Act, is for an applicant of an aggregate permit or licence to notify landowners within 120m of a proposed pit or quarry, to post the notice in a newspaper and to post on a sign at the proposed site (licence only).

The Aggregate Resources Act (ARA) provides for the transfer of a licence or permit. A transferred licence or permit will allow the continued operation of the licensed or permitted site under the same terms, conditions and site plan requirements as the original licence or permit. Any changes or amendments to the site plan or licence/permit conditions are a separate process.

The purpose of this Application Form is to provide MNRF with the necessary information to process and review a request for a new (Type A), revised (Type B) or renewed (Type C) license for Cage Aquaculture Facilities.

Individuals or farming businesses that wish to excavate small amounts of aggregate from a pit on private land without a licence as per section 7(1.1) of the Aggregate Resources Act (ARA) must register the activity with the ministry using this form. All requirements set out in section 7.8 of Ontario Regulation (O. Reg) 244/97 must be met in order for a person to be eligible for this exemption. Only sites on private land, designated under Schedule 1 of O. Reg. 244/97, are eligible to register using this form. Aggregate excavation activities on Crown land are not eligible. Regardless of your eligibility for an exemption under the ARA, other legislation, policy or land use planning approvals may apply (e.g., Planning Act, Municipal Act, Endangered Species Act, Niagara Escarpment Planning and Development Act). It is your responsibility to ensure that you meet requirements and/or obtain appropriate approval(s) under other acts, regulations or policies. Records must be kept for the duration of the operation of the pit and for a period of seven years after the date on which rehabilitation is completed, and must include: 1) copies of all materials that were registered with the Minister and a municipality, 2) documentation demonstrating that all requirements of section 7.8 of O. Reg 244/97 have been satisfied, and 3) detailed records of the quantity and type of material removed from the pit. Any record must be produced for an inspector upon request. Pursuant to subsection 57(5) of the ARA. Note: All eligibility requirements and conditions set out in section 7.8 of Ontario Regulation 244/97 under the Aggregate Resources Act apply. Where there is a conflict between the requirements noted in this form and the regulation, the regulation shall prevail. Form should be submitted through the Natural Resources Information Portal (NRIP).
