This form is part of the Director's Directions for Operational Plans. These directions provide instructions respecting the preparation and content of operational plans prepared by owners of municipal residential drinking water systems pursuant to subsection 16(2) of the Safe Drinking Water Act, 2002, and where otherwise required under the Act. A completed copy of this form is required to be included in every operational plan.

This form supplies to MNR the necessary information for a Minister's decision on the issuance of an overall benefit permit under section 17 of the Endangered Species Act. The combined information provided in the Information Gathering Form (IGF), Avoidance Alternatives Form (AAF) and this permit application form (C-PAF) will be considered by the Minister in forming a permit issuance decision.

The purpose of this Avoidance Alternatives Form (AAF) is to assist the proponent with considering alternatives that would not adversely affect protected species or habitat(s). The completion of the Avoidance Alternatives Form (AAF) by the proponent is required prior to applying for an overall benefit permit (i.e., prior to filling out the Application for an Overall Benefit Permit under clause 17(2)(c) of the Endangered Species Act.

This form is to be used by the primary or alternate account representative (PAR or AAR) of a participant registered under the Climate Change Mitigation and Low Carbon Economy Act, 2016 in order to request approval for the following actions taken in the Compliance Instrument Tracking System Service (CITSS): correct or update information of an existing facility; or add a new facility that has never been entered into the CITSS.

This form is to be used by the primary or alternate account representative (PAR or AAR) of a participant registered under the Climate Change Mitigation and Low Carbon Economy Act, 2016 to request Director approval for the following actions taken in the Compliance Instrument Tracking System Service (CITSS): Request to Change/Add Account Representatives and/or Account Viewing Agents

This form is to be used by the primary or alternate account representative (PAR or AAR) of a participant registered under the Climate Change Mitigation and Low Carbon Economy Act, 2016 to notify the Director of any changes in company or facility ownership pursuant to O. Reg 144/16 The Cap and Trade Program (Cap and Trade Regulation) made under the Climate Change Mitigation and Low Carbon Economy Act, 2016.

This form is to be used by all accredited verification bodies (AVBs) prior to completing the verification of a GHG report under O. Reg. 390/18 to assess the potential for compromised impartiality.

This report must be prepared 6 months before becoming subject to O. Reg. 102/94 and a copy retained on file for at least five years after it is prepared, and be made available to the ministry upon request.

To facilitate the re-registration of a facility into the Ontario Greenhouse Gas Emissions Peformance Standards program.

Form used to register waste generation activity. Use of this form is only permitted if the Director has approved a request to satisfy submission requirements by using a paper document.

This search request form is required to obtain copies of individual water well records i.e. a copy of the actual Well Record as submitted by the Well Contractor at the time of construction.

This form is to be used by persons who are eligible to apply and who wish to apply for registration as a voluntary participant in the cap and trade program pursuant to Part IV of O.Reg 144/16 The Cap and Trade Program Regulation made under the Climate Change Mitigation and Low-carbon Economy Act, 2016 (the “Cap and Trade Regulation”).

Use of this form is only permitted if the Director has approved a request to satisfy submission requirements by using a paper document.
