This form will be used to evaluate requests for the use of site-specific meteorological data under section 13 of Ontario Regulation 419/05: Air Pollution - Local Air Quality (the Regulation) made under the EPA

This form will be used to collect information relating to measurerd or modelled air-related exceedances as required by s.25(9), s.28(1) and s.30(3) of Ontario Regulation 419/05: Air Pollution - Local Air Quality (the Regulation) made under the EPA

This form will be used to evaluate requests for the use of a specified dispersion model under s. 7 of Ontario Regulation 419/05: Air Pollution - Local Air Quality (the Regulation) made under the EPA

This form will be used to evaluate requests for approval of a plan for the combined assessment of modelled and monitored results including the use of meterological data for the purpose of refining emissions rates used in the analysis under paragraph 3 of s.11(1) of Ontario Regulation 419/05

This form is to be used by all accredited verification bodies (AVBs) prior to completing the verification of a GHG report under O. Reg. 390/18 to assess the potential for compromised impartiality.

This form is required to be used by accredited verification bodies (AVBs) to provide a written declaration that attests to whether or not there is a reasonable level of assurance that: 1) a GHG report contains no material discrepancy; and, 2) the report was prepared in accordance with the regulation.

Reg 144/16 The Cap and Trade Program (Cap and Trade Regulation) made under the Climate Change Mitigation and Low Carbon Economy Act, 2016.

This form is completed by a proponent and submitted to the director of the Environmental Assessment Branch. It provides information about the proponent and its proposed undertaking, which is used to create a posting for the Environmental Assessment page on the Government of Ontario's website.
