A plant milk grader certificate is required to grade and receive raw cow milk and raw goat milk delivered to dairy processing plants in Ontario.

A licence is required in order to operation a Fish processing operation that processes or packages fish products at a fish processing plant. A Fish processing plant could be a structure, boat, vehicle or vessel where a fish processing operation operates.

A licence is required to sell livestock medicines to owners of livestock or to another person acting on the owner's behalf, for the treatment of the owner's livestock under the Animal Health Act, 2009 and the Licences to Sell Livestock Medicines Regulation.

A License is needed if you wish to operate a supply facility. Supply facility means premises that are used for breeding and rearing of animals to be used in research, teaching or testing. Please complete this application for a license to operate a supply facility under the Animals for Research Act. A license expires on the 31st day of December of the year of issue. Licenses must be renewed annually. A separate application and payment of fees is required for each supply.

For non-emergency, on-farm slaughter, a certificate is required to provide stunning, slaughter and dressing services, perform ante mortem and post mortem examinations and ensure humane animal handling and sanitary dressing.

This Meat Inspection Program Discussion Request Form is intended for use when further discussion is requested by producers/consigners and/or herd/flock veterinarians regarding condemnations or dispositions made at provincially licensed meat plants in Ontario and information included on Form 202: Whole and/or Partial Carcass(es) Condemnations.

A licence is required for a person in the business of receiving, collecting and/or transporting deadstock. Once the application and supporting documents are completed and submitted, the ministry will review. After it is approved, a licence will be mailed to the applicant.

The licensee of a fish processing operation may consent to the revocation of their licence by a director if the operation no longer conducts regulated activities.

No person shall make an alteration with respect to a meat plant unless the operator of the plant has given written notice of the proposed alteration to a director at least 30 days before making the alteration.

A new licence is required prior to beginning the operations of a dairy plant including: When a facility is being licensed for the first time. When an existing licensed plant is transferred to a new owner (transfer of ownership) An "Application for a Permit to Construct or Alter a Building Intended for Use as a Dairy Plant – For New Applicants" must be submitted at the same time as an “Application for a Licence for the Operation of a Plant”. Note, the licence will not be issued until the plant is complete and the Ontario Ministry of Agriculture, Food and Rural Affairs (OMAFRA) has confirmed, through inspection, that it meets all applicable regulations under the Milk Act (Ontario).

An application is required to make changes to the species of milk used, type of plant, milk class or subclass at a licenced dairy plant receiving cow milk. Details of these changes are provided in Regulation 761, s.96(2) under the Milk Act (Ontario). If these changes require alterations or modifications to a plant you must also submit an “Application for a Permit to Construct or Alter a Building Intended for use as a Dairy Plant - For Existing Applicants”.

Prior to beginning construction of a new dairy plant an "Application for a Permit to Construct or Alter a Building Intended for Use as a Dairy Plant – for New Applicants" must be completed and submitted and approved by the Director. This application must demonstrate, through drawings and other information as necessary, that the plant will be built to meet the regulatory requirements under the Milk Act (Ontario). An “Application for a Licence for the Operation of a Plant” must also be submitted with this permit application.
